3717-1-04.1(KK) / Food equipment - certification and classification.
Repeat Equipment not approved by a recognized testing agency. Observed crockpot,griddle, refrigerator/freezer unit, etc. that is not approved commercial equipment. PIC stated some items are not used. Items that are longer in use shall be removed from the facility.

3717-1-04.5(D) / Nonfood-contact surfaces - cleaning frequency.
Repeat Non-food contact surface not cleaned at the required frequency. Observed the door/door seal of the warewashing machine not maintained clean. This equipment is currently being used as an air drying rack.

3717-1-06.4(A) / Repairing.
Physical facilities not maintained in good repair. Observed stained ceiling tiles in the kitchen. PIC stated there is a leak when it rains.
