Comments
In order for a FSO (Food Service Operation) License to be issued, the following must be completed:
Remove the carpet in the preparation area. Install proper floor tile that can be easily cleaned. A coved base molding is required to ensure floor and wall junctures are easily cleanable.
Tea preparation counter shall be caulked (create a coved finish), provided with a smooth finish on the inside, and all exposed wood shall be sealed with a polyurethane to water proper the untreated wood. If counter tops and shelving is not movable, the equipment shall be fixed to the wall and sealed/caulked.
Finish installing the floor tiles in all locations missing tile.
seal concrete patch under the 3 compartment sink.
Dumpster shall be stored on a paved surface.
Provide a Diarrheal and vomiting procedure for review by the Health department.
Provide a copy of the Certificate of Occupancy for the facility.
Signed application (will be provided once a final approval has been giving) and payment for the amount of $263.00.
Provide and updated menu including the smoothies and what ingredients are planned to be used in each item.
Provide sources for all the herbs. Herbs shall all be properly labeled.
