Comments
In order for a Food Service Operation license to be issued, the following must be completed:
1) Provide a indirect connection on both drainage pipes from the ice machine (2 inch air gap). There should be two drainage lines from the ice machine (one from the maker the other from the Bin).
2) Repair or replace the damaged floor tiles and door threshold to the mop room to be smooth durable and easily cleanable.
3) Provide a New ice bucket that is not damaged/cracked
4) Provide a properly located working temperature measuring device in the walk-in cold holding unit. All cold holding equipment shall be supplied with internal temperature device even with the digital units built in.
5) Provide approval from the building/fire that the fire safety equipment over the cooking area is to code or approved.
6) Once a new ware washing machine installed, provide the make and model number of the unit, must be installed under permit from building dept. must be properly air gapped, etc.
7) Paint the walls to in the stair well to the basement. Recommend installing FRP or similar material.
8) Provide 50 foot candles of lighting over the preparation line. Installing additional light that is shatter-proof or rearrange the set-up to accommodate this requirement.
9) Provide a Signed application and proper payment for the class 4 FSO license.
