This Food service Operation has undergone a final pre-licensing inspection and will be approved for licensing under once the following conditions are completed:
1) Provide a copy of the Final Building and Fire inspection approving the facility to operate. Provide a copy of the Certificate of Occupancy once received from building/fire.
2) Remove noncommercial shelving and install commercial grade shelves for proper air drying over the 3 compartment sink.
3) The shelving over the food preparation cooler is acceptable for now but, will be required to be replaced with commercial grade shelving is not maintained clean or in good condition.
4) Provide a copy of the Person In Charge level 1 certification for at least one person responsible for this food service operation.
5) Provided a signed food service operation application and Risk Classification III license fee of $428.00.

The Application will be provided once the above items have been completed. A License will be issued (available for pick at the health department office or mailed to the mailing address) once a signed application and fee has been submitted.

Contact the Health Department once the above items have been completed. Once the above items have been completed, the FSO is permitted to operate.
