Repeat Non-food contact surfaces of equipment are unclean (cold holding equipment, cooking equipment, storage racks and shelves).

3717-1-06.4(B) / Cleaning - frequency and restrictions.
Repeat Facility not maintained clean. Floors and walls are not clean in the kitchen in food preparation areas.

3717-1-06.4(A) / Repairing.
Repeat Physical facilities not maintained in good repair. There are multiple surfaces in the kitchen that are in disrepair (floors, walls, ceilings, coved base). Surfaces are no longer smooth, durable, and easily cleanable.

Repeat Light intensity less than fifty foot candles in food preparation areas.

The following violation(s) have been corrected since the last inspection.
3717-1-02.4(C)(14)/PIC: duties - ensure employees are properly trained in food safety, including food allergy awareness.
Person in charge did not ensure their employees are properly trained in food safety. Observed multiple critical violations (cleaning, date marking, raw egg storage, pests) at time of inspection indicating a lack of proper employee training. Education was provided to improve food safety.
3717-1-04.5(A)(1)/Cleanliness of equipment food-contact surfaces and utensils.
Equipment food-contact surfaces or utensils are unclean (dough equipment). Education was provided to improve cleaning procedures.
3717-1-03.4(G)/Ready-to-eat, time/temperature controlled for safety food - date marking.
Refrigerated, ready-to-eat, TCS foods not properly date marked (ham, tomatoes, rice). Education was provided to improve date marking procedures.
3717-1-06.4(K)/Controlling pests.
Presence of live insects (flies) in the kitchen. Education was provided to reduce fly population.
3717-1-06.1(M)/Outer openings - protected.
Outer opening not protected. Multiple exterior doors were left propped open to the kitchen for an extended period of time during inspection allowing for potential pest intrusion.
3717-1-03.2(K)/In-use utensils - between-use storage.
In-use utensils improperly stored. Scoop for rice was stored with handle in contact with food.
3717-1-05.4(P)/Maintaining refuse areas and enclosures.
Improperly cleaned storage area for refuse. The trash dumpster was overflowing without proper lids and loose litter was scattered nearby.

Note: The re-inspection was delayed until 7/21/23 at the direction of the Environmental Health Director, because the PIC requested more time to correct violations.
Note: The used cooking oil is currently stored in a container near the parking lot on soil.
