3717-1-04.1(KK) / Food equipment - certification and classification.
Repeat Observed equipment (blender, microwave, 2 refrigerators/freezers, crock-pot, handheld mixer, roaster) at the time of inspection that was not approved by a recognized testing agency.
To prevent health hazards, food equipment that is acceptable for use in a FSO shall be approved by a recognized food equipment testing agency. Unapproved equipment shall be removed or replaced with commercial equipment.
3717-1-04.4(A)(1) / Equipment - good repair and proper adjustment.
Observed the cold holding equipment ( #1) closest to the back wall not maintained in good working order at the time of inspection.
To ensure foods are being held at the proper temperature, cold holding equipment shall be maintained in a state of repair and condition that meets the requirements specified under rules 3717-1-04 and 3717-1-04.1 of this code. Repair or replace this unit.
3717-1-06.4(J) / Using dressing rooms and lockers.
Observed employee belongings and lunches stored among the operations items at the time of inspection.
To prevent contamination, lockers or other suitable facilities shall be used for the orderly storage of employee clothing and other possessions. Discontinue storing personal belongings throughout the facility.

The operation was currently only providing pre packaged snacks at the time of inspection.
Employee food shall be kept separate from the operation. Recommend designating space and shelving.
Breakfast and Lunches were provided by an exempt child food program (Community Action Council of Portage County, Inc). The program receives prepared foods from innovation food service.
The violations documented on this report will be checked during next standard inspection.
