Comments
In order for an Retail Food Establishment license to be issued, the following must be completed:
1) Provide documentation that the grease interceptor has been service and inspected by a licensed plumber.
2) Provide documentation that the main backflow prevention device has been tested and functional.
3) Provide documentation from building department regarding the approval of alterations to the Ansel system.
4) The hood system shall be professionally cleaned.
5) Repair the missing and damaged tiles on the front pass through counter. Surfaces shall be smooth, durable and easily cleanable.
6) Thoroughly clean the floors, walls, ceilings equipment (inside and out) to be "brand new" clean.
7) Provide hot-water (100 degrees F) at the hand washing sink in the employee restroom.
8) Provide a sign on front door regarding no public rest-rooms.
9) Paper towels, soap, hand washing signage (employees much wash hands) shall be provided at each hand sink.
10) Re attach the sink vanity to wall in rest-room.
11) Outdoor grease container shall be installed on the concrete pad.
12) Refuse dumpster concrete pad is cracked and damaged. Plan to have this replaced at some point.
13) Provide documentation verifying the training of the employees regarding the reporting their health issues to the PIC.
14) Provide a signed application (provided for you at the next inspection) and license fee for the amount of $500.
15) If any foods or sauces are to be processed at the Brown Street location in Akron, the Akron location shall registered with the Ohio Department of Agriculture to be a processing location.
