Critical Repeat At the time of inspection, observed there was no handwashing sink located behind the left side bar.
A handwashing sink shall be located to allow convenient use by employees in food preparation, food dispensing, and warewashing area, and in, or immediately adjacent to, toilet rooms. Correct By: 02-May-2014

Critical At the time of inspection, observed a direct connection between the sewage system and a drain originating from the ice machine bin..
A direct connection may not exist between the sewage system and a drain originating from equipment in which food, portable equipment, or utensils are placed unless allowed by the Ohio building code. To protect ice from contamination, eliminate this direct connection. Correct By: 04-Apr-2014

Critical At the time of inspection, observed handwashing sink at the right side bar was supplied with hand sanitizer, but no supply of hand cleaning liquid, powder or bar soap.

Critical At the time of inspection, observed one cloth towel at the handwashing sink for the right side bar.

Comments
The beverage cooler at the right side bar was moved to provide better accessibility to the handwashing sink. Interior surfaces of the walk in cooler were smooth, durable, and clean at the time of inspection. Spray bottles are labelled "sanitizer" Ensure that equipment is sanitized for 60 seconds at a concentration that is between 200 ppm and 400 ppm when a quaternary ammonia sanitizer is used.
A facility review is required before installing the handwashing sink behind the left bar of the facility. Please review the facility review packet provided at the time of inspection, and contact the City of Kent Health Department if there are any questions. Plumbing alterations should be made only by a licensed plumber.
