3717-1-04.5(A)(1) / Cleanliness of equipment food-contact surfaces and utensils.
Critical Observed food-contact surfaces of the ice machine scoop were dirty at the time of inspection.
To prevent contamination, equipment food-contact surfaces and utensils shall be clean to sight and touch. Clean and sanitize the ice scoop or replace it new. Ensure that the ice scoop is stored in a manner that protects it from contamination. Correct By: 11-Aug-2015

3717-1-04(G) / Wood - use limitation.
Observed unapproved food contact surfaces of some bowls and plates contained wood at the time of inspection.
Because of deterioration and cleaning concerns, wood and wood wicker may not be used as a food-contact surface. Remove from the facility all food equipment that contains or is constructed of wood.
3717-1-04.1(H) / Nonfood-contact surfaces - cleanability
Observed nonfood-contact surfaces (underside of shelves above the prep cooler) were not designed and constructed to allow easy cleaning or maintenance at the time of inspection.
To protect against contamination, nonfood-contact surfaces shall be designed and constructed to allow easy cleaning and to facilitate maintenance. Remove all peeling paint from the shelving surfaces so that they are maintained smooth and easily cleanable.
