3717-1-06.4(K) / Controlling pests.
Critical Observed mouse feces in the back corner of the dry storage area that indicated the presence of live rodents in the facility at the time of inspection.
To prevent illness, the presence of insects, rodents, and other pests shall be controlled by: Routinely inspecting incoming shipments of food and supplies; routinely inspecting the premises for evidence of pests; Using methods, if pests are found, such as trapping devices or pesticides used according section (C), (K) and (L) of rule 3717-1-07.1 of this code; and eliminating harborage conditions. A professional pest control agency with licensed personnel shall be hired to eliminate pests in this facility. Correct By: 03-Feb-2015

3717-1-03.2(Q) / Food storage - preventing contamination from the premises.
Observed improper storage of food items (bags of bulk grains, carrots, and other foods in storage or on display) on or within 2 inches of the floor at the time of inspection.
Food shall be protected from contamination by storing the food in a clean, dry location, where it is not exposed to splash, dust, or other contamination, and at least six inches above the floor. Correct By: 03-Feb-2015
3717-1-05.4(B) / Outdoor storage surface.
Repeat Observed the outdoor refuse storage surface for the refuse container was not properly constructed, and sloped to drain at the time of inspection.
A properly constructed storage surface that can be thoroughly cleaned is less likely to attract pests. Provide an outdoor storage surface for refuse, recyclables, or returnables that is constructed of nonabsorbent material and is smooth, durable, and sloped to drain.

Inspection was conducted in response to an anonymous report that mice were present in this facility. Observed the presence of a live mouse inside a humane trap at the time of inspection. Observed mouse feces in the corner of the dry storage area. Several bags of bulk grain foods were examined. These bags appeared to have no tears or chew marks at the time of inspection, however food stored in a container that is discovered to have tears, chew marks, or other evidence of contamination shall be discarded. Discussed the health risks, including specific illnesses that may be transmitted by rodent pests and ways to protect food from contamination. By the time of the follow up inspection, establishment must provide proof that a professional pest control agency with licensed personnel has been hired for the elimination of pests in this facility.
