[?] A summary of the violations found during the inspection are listed below.

3717-1-06.1(M) / Outer openings - protected.
Repeat Outer opening not protected. Observed no self-closing and self-latching back door used for receiving foods and taking out trash.

3717-1-02.2(G) / Jewelry - prohibition.
Food employees wearing jewelry on arms or hands during food preparation.

3717-1-04.1(KK) / Food equipment - certification and classification.
Equipment not approved by a recognized testing agency. Observed non-commercial containers being used throughout the facility (rubbermaid, tupperware, etc.) Documentation stating containers are NSF or equivilent is needed for some containers. Continue to work on replacing containers. Damaged/cracked containers shall be replaced.

3717-1-04.1(H) / Nonfood-contact surfaces - cleanability
Nonfood-contact surfaces not easily cleanable. Observed wood plank on the floor in the walk-in that is not cleanable and shall not be stored on the floor to allow for the floor to be cleaned easily.

3717-1-04.5(A)(3) / Cleanliness of nonfood-contact surfaces of equipment.
Non-food contact surfaces of equipment are unclean. Observed an accumulation of food residue or other debris on the walk-in shelving units and oil mixture containers under finish table.

3717-1-05.1(S) / Plumbing system - maintained in good repair.
Repeat Plumbing system not properly maintained. Observed sewer-like odor in the back storage room near the water heater and prep sink floor drain. Floor drain appeared to have grease built up around the drain line.

3717-1-06.4(A) / Repairing.
Physical facilities not maintained in good repair. Observed ceiling tiles above the front service counter being held up by cardboard and paint stirs. Observed damaged ceiling in this area.

Comments
A food thermometer shall be readily available at all times. Ensure thermometers are being tested for accuracy and calibrated if needed.
Continue to repair walls and surfaces that are no longer smooth and easily cleanable.
Handwashing sinks shall be properly supplied at all times.
A new prep refrigeration unit has replaced the old equipment. Delfield sandwich prep table. Provide model number.
The prep room has been rearranged and prep tables and other equipment have been removed. Excess equipment is being stored near the back door and shall be there only temporarily.

The violations documented shall be corrected and will be checked during next standard inspection.
