Repeat Facility not maintained clean. Walls near the fryer are not clean. Floors are not clean in the kitchen under equipment. Floors of the basement are not clean.

3717-1-06.4(N) / Maintaining premises - unnecessary items and litter.
Repeat Unnecessary or nonfunctional items stored in the basement (old equipment). The excess storage may allow pest harborage.

3717-1-02.4(C)(16)/PIC: duties - ensure written procedures and plans are maintained and implemented in the facility.
Written procedures and plans are not maintained and implemented. The FSO was using an unapproved TILT (time in lieu of temperature) policy for preparing fries. The PIC was not knowledgeable about a proper TILT procedure and the FSO does not have a risk class 4 license to permit this procedure. Education was provided to the PIC about how to comply going forward.
3717-1-02.4(B)(2)(h)/PIC: Demonstration of Knowledge - Describing relationship between prevention of foodborne illness and management of contamination, hand contact with food, hand washing, and clean facility
PIC unable to demonstrate knowledge of preventing foodborne illness by controlling contamination, proper hand washing, and cleaning. Observed multiple violations (hand washing, cross contamination, cleaning) that could lead to food borne illness. Education was provided to the PIC to improve food safety.

Person in charge did not ensure their employees are properly trained in food safety. Observed multiple violations in the FSO (date marking, raw meat storage, hand washing, TILT, pests, cleaning, basement storage, etc.) indicating a lack of proper food safety amongst the kitchen staff. Education was provided to improve food safety going forward.
3717-1-02.2 (C )/Hands and arms: when to wash
Food employee(s) did not wash hands when required. The food preparation employee failed to wash hands after contacting raw eggs and before contacting RTE foods when changing gloves. Education was provided to the PIC about proper hand washing procedures.

Food not properly protected from contamination by separation, packaging, and segregation. Raw steak and raw eggs were stored over RTE foods in the preparation cold holding unit. Education was provided to the PIC about proper storage.

Equipment food-contact surfaces or utensils are unclean. Cooking utensils (spatulas) used at the grill are not clean. Education was provided to the PIC about proper ware washing procedures.

TCS foods not being hot held at the proper temperature. Fries were holding at 66 degrees F near the fryer. Education was provided to the PIC about how to properly use a TILT policy (time in lieu of temperature).

Refrigerated, ready-to-eat, TCS foods not properly date marked (lettuce). Education was provided to the PIC to improve date marking procedures.

Improper storage of poisonous or toxic materials. A chemical spray bottle was stored over spice near the hand washing sink. Education was provided to the PIC about proper storage.

Observed a dead cock roach on a monitoring station in the basement. Education was provided to the PIC about proper pest control.
