Comments
This FSO has undergone a final pre-licensing inspection and has been approved to be issued a license with the understanding that the following will be completed:
1) Caulk or install coved base molding along the floor and wall junctures of the front bar
2) Caulk or install coved base molding along the floor and cabinet juncture next to the beer tap cabinet
3) Caulk or install coved base molding along the floor and wall junctures through out the back hallway.
4) Caulk or install coved base molding along the floor and bar wall juncture under the server drink station
5) Recommend not using brick under the ice machine. Research another solution that would be considered smooth, durable and easily cleanable. Complete the install of water lines to the ice maker.
6) Provide additional lighting at the Prep table/preparation sink area. At least 50 foot candles will be required.
7) Provide a procedure/policy regarding the future garage door use and pest issues.
8) Per the FDA guidelines, the tankless hot water supply is less than what is required. Recommend providing an additional tankless hot water heater.
9) Provide documentation that the the grease interceptor was serviced and properly functioning.
10) Provide a Certificate of Occupancy once provided by the Fire Department/Building Department.

Owner stated that the above items will be completed before the FSO operates. If the above items are not completed by the 30 day inspection, all shall be documented according to the Ohio Food Code.

Okay to operate once a signed application and license fee for the amount of $630 has been submitted to the Kent City Health Department.
