[?] A summary of the violations found during the inspection are listed below.

3717-1-03.4(F)(1) / Time/temperature controlled for safety food - hot and cold holding.
Critical Observed TCS foods were not being held at the proper temperature. Observed milk to be holding a unacceptable temperatures at the time of inspection.
Except during preparation, cooking, or cooling, or when time is used as a public health control, TCS food shall be held at 135°F or above (except for those roasts that are cooked as required may be held at 130° F), or at 41°F or less. All foods that require refrigeration shall be properly held at 41°F or below to limit the growth of harmful organisms.
3717-1-05.1(O)(2) / Using a handwashing sink - other uses prohibited.
Critical Handwashing sink is being used for purposes other than handwashing. Observed hand sink is being used to store sanitizer and wash buckets at the time of inspection
A handwashing sink may not be used for purposes other than handwashing to prevent possible contamination.
3717-1-05.3(C) / Backflow prevention.
Critical Repeat Observed a direct connection between the sewage system and a drain originating from equipment (food preparation sink) in which foods are placed..
A direct connection may not exist between the sewage system and a drain originating from equipment in which food, portable equipment, or utensils are placed unless. Provide proper back flow prevention on the sink used for washing ready to eat foods. Until this change is made, all food shall be washed at the commissary kitchen (Middle School).

3717-1-04.1(KK) / Food equipment - certification and classification.
Observed in-use pasta container was not approved by a recognized testing agency. Observed pasta to be delivered from Staton Middle School in a unapproved storage container at the time of inspection
Food equipment that is acceptable for use in a FSO or RFE shall be approved by a recognized food equipment testing agency. Foods shall on contact approved food grade equipment and utensils. Discontinue using streline containers for storing foods.
3717-1-04.4(A)(1) / Equipment - good repair and proper adjustment.
Observed milk cold holding equipment and/or components are not maintained in good working order at the time of inspection.
Equipment shall be maintained in a state of repair and condition that meets the requirements specified under rules 3717-1-04 and 3717-1-04.1 of this code. Repair or adjust milk cooler to hold milk at acceptable temperatures.
3717-1-04.4(S)(1) / Single-service articles and single-use articles - use limitation.
Observed re-use of single-service black bowls used for head start program) or single-use articles at the time of inspection.
Single-service and single-use articles may not be reused and shall be discarded after use. Only food grade approved containers shall be used.
3717-1-06.1(I) / Light bulbs - protective shielding.
Repeat Observed light bulbs directly of the kitchen were not properly shielded or coated at the time of inspection.
Light bulbs shall be shielded, coated, or otherwise shatter-resistant in areas where there is exposed food, clean equipment, utensils, or linens, or unwrapped single-service or single-use articles and does not apply in areas used only for storing food in unopened packages. An infrared or other heat lamp shall be protected against breakage by a shield surrounding and extending beyond the bulb so that only the face of the bulb is exposed.
3717-1-06.1(M) / Outer openings - protected.
Repeat Observed an window without proper protection against the entry of insects and rodents. Observed the left window in the kitchen was without a proper screen at the time of inspection.
To prevent contamination, outer openings of an FSO or RFE shall be protected against the entry of insects and rodents as specified in this rule. Provide a screen (Sixteen mesh to one inch) on the left and right window with in the kitchen.
3717-1-06.2(D) / Handwashing aids and devices - use restrictions.
Repeat Observed soap and towels for handwashing at the utensil washing sink at the time of inspection.
A sink used for food preparation or utensil washing, or a service sink or curbed cleaning facility used for the disposal of mop water or similar wastes, may not be provided with handwashing aids and devices required for a handwashing sink.
