Critical Corrected During Inspection Equipment food-contact surfaces or utensils are unclean. The dirty deli slicer was properly cleaned by the PIC once the violation was identified.

3717-1-04.4(A)(2) / Equipment components kept intact, tight, and adjusted
Equipment components are not intact, tight or properly adjusted. The main pizza preparation cold holding unit had an interior leak.

3717-1-04.5(A)(3) / Cleanliness of nonfood-contact surfaces of equipment.
Non-food contact surfaces of equipment are unclean. Multiple pieces of equipment were dirty in the kitchen (cold holding equipment, dough equipment, cooking equipment, storage racks and shelving).

Repeat Floors throughout the facility not maintained clean (basement, restroom, mop room, dough area, chicken area, pizza area, ware wash area, and the salad preparation area).
