3717-1-02.4(C)(15) / PIC: duties - ensure employees are informed in a verifiable manner of their responsibility to report their health information
Critical Corrected During Inspection Employees are not informed in a verifiable manner of their responsibility to report information about their health. PIC had the 2 employees working read the FSO's employee health reporting agreement and sign. PIC stated all other workers have signed the form.

3717-1-03.2(C) / Packaged and unpackaged food - preventing contamination by separation, packaging, and segregation
Critical Corrected During Inspection Food not properly protected from contamination by separation, packaging, and segregation. Observed raw shell eggs stored on the top shelf in the walk-in above RTE foods. Observed personal belongings stored throughout the walk-in. PIC removed the eggs and other personal belongings and placed them in a designated location.

3717-1-03 / Food - safe, unadulterated, and honestly presented
Food was unsafe, adulterated, or not honestly presented. Observed dented cans of nacho cheese sauce. PIC stated the supplier will not give credit and have to use the dented cans. PIC stated they will not use the can and was unaware of safety concern of using dented cans.

3717-1-03.2(D) / Food storage containers - identified with common name of food.
Working food containers throughout the facility not properly labeled.

3717-1-06.1(M) / Outer openings - protected.
Repeat Outer opening not protected. Observed no self-closing and self-latching back door used for receiving foods and taking out trash.

3717-1-02.2(G) / Jewelry - prohibition.
Food employees wearing jewelry on arms or hands during food preparation.

3717-1-04.5(A)(3) / Cleanliness of nonfood-contact surfaces of equipment.
Non-food contact surfaces of equipment are unclean. Observed an accumulation of food residue or other debris on the walk-in shelving units, oil mixture containers under finish table, and countertop area around front handwashing sink.

3717-1-06.1(A) / Floors, walls, and ceilings.
Floors, walls, and/or ceilings not smooth and easily cleanable. Observed damaged ceiling above the front service counter. Observed damaged walls throughout the facility. Observed damaged counter top at the customer service drink station. Observed damaged floor under and around the main prep refrigeration unit.

3717-1-06.4(A) / Repairing.
The flooring around the grease interceptor is damaged and not properly sealed. PIC sealed the lid on the grease trap at the time of inspection.

Comments
Observed the facility cooling and reheating the nacho cheese sauce. This procedure requires a risk level 4 license.
The facility is conducted risk 4 activities and will need to increase there license to a risk level 4 for next licensing year.
Proper handwashing shall occur prior to donning gloves after breaking task.
Ensure light intensity is 50 foot candles or more where employees are working with knives and sharp equipment.
Continue to repair walls and surfaces that are no longer smooth and easily cleanable.
Handwashing sinks shall be properly supplied at all times. Observed jammed paper towel dispenser at the hand sink near the three-compartment sink.
Please provide the model number for the Delfield sandwich prep table.

The violations documented shall be corrected and will be checked during next standard inspection.
