[?] A summary of the violations found during the inspection are listed below.

3717-1-02.2(G) / Jewelry - prohibition.
Food employees wearing jewelry on arms or hands (watch and bracelet) during food preparation.

3717-1-04.4(A)(2) / Equipment components kept intact, tight, and adjusted
Equipment components are not intact, tight or properly adjusted. Observed damaged knife and knife case for the turquoise knife on the prep refrigeration unit used for banana's.

3717-1-04.5(A)(3) / Cleanliness of nonfood-contact surfaces of equipment.
Shelving above the three compartment sink, air drying rack, and the faucet for the hand sink were not maintained clean. Shelving appeared to be in poor condition and shall be replaced if unable to clean.

3717-1-06.4(A) / Repairing.
Cracked floor tiles throughout the facility, specifically near back door and prep refrigeration unit. Flooring is no longer smooth and easily cleanable. Observed ceiling stained and damaged throughout and in need of cleaning.

3701-21-25(I) / Person in charge certification in food protection
FSO did not have a person in charge per shift with the person in charge certification in food protection.

Employee belongings shall be kept separate in a designated location away from the FSO's items.
Ensure opened containers of milk are date marked if the product is not used within 24 hours.
Bottom shelves of shelving units shall be installed at least 4 inches off the floor.
The violations documented shall be corrected and will be checked during next standard inspection.
