3717-1-03.5(E) / Consumer advisory.
Critical Repeat Observed that the consumer is not properly advised of the risk of consuming raw or undercooked animal foods because the consumer advisory was incomplete for some menu items at the time of inspection.
Except for whole-muscle intact beef steaks, if an animal food such as beef, eggs, fish, lamb, milk, pork, poultry, or shellfish is served or sold raw, undercooked, or without otherwise being processed to eliminate pathogens, either in ready-to-eat form, or as a raw ingredient in another ready-to-eat food, the license holder shall inform consumers of the significantly increased risk of consuming such food by way of a disclosure and reminder as described in this rule.
3717-1-04.5(A)(1) / Cleanliness of equipment food-contact surfaces and utensils.
Critical Observed food debris on food-contact surfaces of the deli slicer at the time of inspection.
To prevent contamination, equipment food-contact surfaces and utensils shall be clean to sight and touch. The deli slicer shall be kept clean between uses.

3717-1-04.4(A)(2) / Equipment components kept intact, tight, and adjusted
Repeat Observed equipment components (seals for meat cooler and burger cooler) were not intact at the time of inspection.
Equipment components such as seals shall be kept intact, tight, and adjusted in accordance with manufacturer's specifications. Replace the seals for these coolers so that the equipment continues to function properly.
3717-1-04.5(A)(3) / Cleanliness of nonfood-contact surfaces of equipment.
Observed an accumulation of food residue and debris on the exterior surface and handle of the meat cooler and an accumulation of liquid in the cabinet of the meat cooler at the time of inspection.
Nonfood-contact surfaces of equipment shall be kept clean. Clean the interior and exterior of the meat cooler and provide regular cleaning that maintains this equipment clean to sight and touch.
3717-1-06.1(B) / Floors, walls and ceilings - utility lines.
Observed utility service lines in the kitchen at the time of inspection that were improperly exposed or installed
Utility service lines may not be unnecessarily exposed, and shall be installed so they do not obstruct or prevent cleaning ceilings.
3717-1-06.2(E) / Handwashing signage.
Repeat Observed no handwashing sign posted at handwashing sink used by employees in the upstairs men's restroom at the time of inspection.
A sign or poster that notifies food employees to wash their hands shall be provided at all handwashing sinks used by food employees.
3717-1-06.4(B) / Cleaning - frequency and restrictions.
Observed a build-up of dust on areas of the ceiling in the kitchen, especially on and around utility service lines at the time of inspection.
The physical facilities shall be cleaned as often as necessary to keep them clean, and cleaning shall be done during periods when the least amount of food is exposed. Clean all accumulated dust from the ceiling and utility service lines. Provide regular cleaning that maintains all areas of the facility clean.

Pre-existing non-commercial refrigerator in the facility is approved for use as long as it continues to function properly and is maintained clean. All newly purchased equipment shall be commercial grade. Contact the City of Kent Health Department for approval before placing any new equipment into use.
