3717-1-05.1(O)(1) / Using a handwashing sink - accessible at all times
Critical Corrected During Inspection Handwashing sink not accessible. Rear hand washing sink blocked by milk crates. The PIC immediately moved the items to allow proper access to sink.

3717-1-03.4(H)(1) / Ready-to-eat, time/temperature controlled for safety food - disposition.
Critical Corrected During Inspection Ready-to-eat, TCS food not properly discarded when required. Chicken wings with a preparation date of 1/22 were stored in the preparation cold holding unit. The PIC immediately and voluntarily discarded the items once the violation was identified.

3717-1-03.4(G) / Ready-to-eat, time/temperature controlled for safety food - date marking.
Critical Refrigerated, ready-to-eat, TCS foods not properly date marked (sausage, lettuce, rice, crab, meatloaf, potatoes, brisket). Education was provided to improve date marking procedures.

3717-1-07.1(C) / Poisonous or toxic materials: Conditions of use.
Critical Corrected During Inspection Improper use of a poisonous or toxic substance. An unapproved pesticide was being used at the bar to combat fruit flies. The PIC immediately removed the device once the violation was identified.

3717-1-04.8(E)(1) / Single-service and single-use articles - storage
Cups and to-go containers were stored on the basement floor.

3717-1-04.5(A)(3) / Cleanliness of nonfood-contact surfaces of equipment.
Non-food contact surfaces of equipment are unclean (interior surfaces of cold holding equipment, storage racks and shelving).

3717-1-06.1(R) / Outdoor refuse areas - curbed and graded to drain.
Repeat Outdoor refuse areas were not properly constructed. Dumpster currently placed on gravel parking lot.

3717-1-06.4(A) / Repairing.
Repeat Physical facilities not maintained in good repair. There are multiple roof leaks. There are multiple areas with damaged ceiling tiles, floor tiles, coved base, and missing caulk throughout the facility.

3717-1-06.4(N) / Maintaining premises - unnecessary items and litter.
Repeat Unnecessary or nonfunctional items stored in the basement (old equipment, paint, etc.).

3717-1-06.4(B) / Cleaning - frequency and restrictions.
Floors are not clean in hard to reach areas under equipment. HVAC fans are not clean.

Comments
The FSO is currently going through a Facility Review due to recent bar renovations. Any potential issues at the bar as a result of the renovations will be addressed during the Facility Review.
The PIC stated that their mobile food truck operated during the summer of 2021 without a proper license. Make sure the proper license is obtained for 2022.
