Comments
The following must be completed in order for the Retail Food Establishment to be approved to be issued a license:
1) Provide a self-closing front door. Properly attach the threshold to the floor. Provide the rest room door to be self closing.
2) Caulk around all the door frames in the facility. Provide a smooth and easily cleanable surface.
3) Provide shatter proof lighting over the food preparation line.
4) Repair all the damaged flooring tiles in the facility. By the front door, under the food preparation sink.
5) Remove the household freezer from the facility
6) Install FRP or similar material around the mop sink.
7) All cold holding units must be on and holding at the proper cold holding temperatures.
8) Provide hand washing signage at all the designated hand washing sinks. Employees Must Wash Hands
9) Remove the bulk pizza to go boxes from the front of the facility. No storage for single use articles or foods shall be in public areas.
10) Observed the hot water supply to not be sufficient to meet the hot water demands. Provide a proper hot water supply. Hot water supply was unable to meet the demand to fill up the 3 compartment sink. Provide at least 120-140 Degrees F for ware washing sink. Provide a legal documentation regarding a install date of the new hot water tank. Provide a date when the improvement will be completed.
11) Caulk around the grease trap to create the junctures to be easily cleanable.
12) Provide a mop hanger over the mop sink.
13) Provide the gas connect to be the proper quick disconnection.
14) Provide Final approval from building and Fire department approving the facility to operate. Provide a copy of the final approval report.
15) Provide a copy of the certificate of occupancy.
16) Provide a certificate of Training for the Person In Charge.
