[?] A summary of the violations found during the inspection are listed below.

3717-1-05.3(C) / Sewage and other liquid waste: Backflow prevention.
Critical Repeat Observed a direct connection between the sewage system and a drain originating from equipment in which food, portable equipment, or utensils are placed. Observed the facility was not provided with a designated, properly installed sink for preparing ready to eat foods at the time of inspection.
To prevent health hazards, a direct connection may not exist between the sewage system and a drain originating from equipment in which food, portable equipment, or utensils are placed unless allowed by the Ohio building code. Provide a food preparation sink that is properly installed with a in directly connected drainage to prevent contamination to foods. Until this sink is provided, all foods that need to be washed shall be washed at the Stanton Middle school kitchen.

3717-1-02.4(A)(2) / Person in charge: assignment of responsibility.
Observed facility did not have one employee with supervisory and management responsibility and the authority to direct and control food preparation and service with level two certification in food protection at the time of inspection.
To prevent health hazards, at least one employee that has supervisory and management responsibility and the authority to direct and control food preparation and service shall obtain the level two certification in food protection according to rule 3701-21-25 of the Administrative Code. Provide proof of level 2 certification for a proper employee.
3717-1-04.4(A)(1) / Equipment - good repair and proper adjustment.
Observed the milk cold holding #1 holding water in the interior of the equipment at the time of inspection.
To prevent health hazards, equipment shall be maintained in a state of repair and condition that meets the requirements specified under rules 3717-1-04 and 3717-1-04.1 of this code. Interior of cold holding equipment shall be cleaned and repaired to further prevent holding water.
3717-1-06.1(M) / Outer openings - protected.
Repeat Observed an outer opening door without proper protection against the entry of insects and rodents. Observed back door to the kitchen was not self-closing at the time of inspection.
To prevent contamination, outer openings of an FSO shall be protected against the entry of insects and rodents as specified in this rule. Provide the back door to kitchen to be self-closing, self latching and tight-fitting.
