[?] A summary of the violations found during the inspection are listed below.

3717-1-04.1(H) / Nonfood-contact surfaces - cleanability
Nonfood-contact surfaces not easily cleanable. Observed wood surfaces at the serving windows in poor condition. Observed waffle bowl container cracked and in poor condition.

3717-1-04.5(A)(3) / Cleanliness of nonfood-contact surfaces of equipment.
Shelving above the three compartment sink, air drying rack, and the faucet for the hand sink were not maintained clean. Shelving appeared to be in poor condition and is to be replaced if unable to be cleaned.

3717-1-06.1(A) / Floors, walls, and ceilings.
Cracked floor tiles throughout the facility, specifically near back door and prep refrigeration unit. Observed some flooring installed in the middle of the kitchen.

3717-1-06.4(A) / Repairing.
Repeat Observed ceiling stained and damaged throughout and in need of cleaning. Unfinished drywall shall be finished to be smooth and easily cleanable.

Bottom shelves of shelving units shall be installed at least 4 inches off the floor.
Ensure documentation can be provided that all employees have signed the operation's sick policy.
Ingredient labels shall be available for all products within the facility, specifically items that are removed from original packaging.
Chemicals are to be stored in a designated location away from clean items and prep surfaces.

The violations documented shall be corrected and will be checked during next standard inspection.
