[?] A summary of the violations found during the inspection are listed below.

3717-1-05.3(C) / Sewage and other liquid waste: Backflow prevention.
Critical Repeat Observed a direct connection between the sewage system and a drain originating from equipment (food preparation sink) in which foods (apples) were washed and prepared at the time of inspection.
To prevent contamination to foods, a direct connection may not exist between the sewage system and a drain originating from equipment in which food, portable equipment, or utensils are placed. Provide proper back flow prevention on the designated food preparation sink. All work shall be completed by a licensed plumber and under permit. Until this sink has been provided with a proper air gap, all foods shall be delivered to the FSO washed from the supplier (Stanton Middle School).

3717-1-04.4(P) / Warewashing equipment - determining chemical sanitizer concentration.
Observed the sanitizer concentration was not being monitored with a test kit or other device. Observed the sanitizer solution was well over 500 PPM in the sanitizer bucket at the time of inspection.
To ensure proper sanitization, the concentration of the sanitizing solution shall be accurately determined by using a test kit or other device. Test the sanitizer solution to ensure the proper amount of sanitizer is being used (200-400 PPM).
3717-1-04.8(E) / Equipment, utensils, linens, single-service articles, and single-use articles - storage.
Observed single-service and single-use articles (boxes of serving trays stored on the floor) were not properly stored at the time of inspection.
To prevent contamination, cleaned equipment and utensils, laundered linens, single-service articles, and single-use articles shall be stored: (a) In a clean, dry location; (b) Where they are not exposed to splash, dust, or other contamination; and (c) At least six inches (fifteen centimeters) above the floor.
3717-1-06.3(A) / Handwashing sinks - conveniently located.
Observed no conveniently located handwashing sink with in the ware washing area and the food preparation area at the time of inspection.
Handwashing sinks shall be conveniently located as specified under paragraph (L) of rule 3717-1-05.1 of this code. To ensure proper hand washing by food employees, additional hand washing sinks shall be provided in ware washing and food preparation area.
3717-1-06.4(B) / Cleaning - frequency and restrictions.
Repeat Observed accumulation for old pest droppings in the back storage room (on and around old walk-in condenser and on electrical components) at the time of inspection.
To prevent contamination, the physical facilities shall be cleaned as often as necessary to keep them clean, and cleaning shall be done during periods when the least amount of food is exposed. Clean the locations described above. If new droppings accumulate, contacted a licensed pest control operator.
3717-1-06.4(D) / Cleaning ventilation systems, nuisance and discharge prohibition.
Observed the ventilation system (hood system over the oven was dusty) was not being properly cleaned at the time of inspection.
Intake and exhaust air ducts shall be cleaned and filters changed so they are not a source of contamination. Clean the vents on the hood system over the oven.
