| This RFE has undergone a final pre-licensing inspection and has been approved to be issued a license with the understanding that the following will be completed by the 30 day inspection: 1) Increase the lighting at the hand washing sink in the rest room to 20 foot candles. 2) Remove/replace the non-commercial mixers with approved commercial (NSF or equivalent). 3) Install a proper back flow prevention device on the direct connection water supply line to the coffee maker. (ASSE 1022) 4) Finalize the warewashing flow. Provide a location to store dirty utensils and equipment to ensure separation from clean. 5) Replace the residential grade gas line to the stove with commercial grade. (Per Building Dept.) 6) Provide a Diarrheal and Vomit clean up kit and procedures. 7) Provide a door to the beverage cooler. Ensure it holds at 41 degrees F or below. 8) Replace and seal the ceiling tiles over the oven. Maintain the fire protection. 9) Provide a copy of the Certificate of Occupancy. 10) ensure single-use items are properly stored. 11) All shelving shall be installed to have the lower shelf at least 6 inches above the floor. These items will be documented according to the code if not corrected by the 30 day inspection. A signed application for a License to conduct a RFE was received along with payment (Check #1506) for the amount of $500. Okay to Operate. License will be mailed once processed to the designed mailing address. |
