[?] A summary of the violations found during the inspection are listed below.

3717-1-03.4(G) / Ready-to-eat, time/temperature controlled for safety food - date marking.
Critical Refrigerated, ready-to-eat, TCS foods (cooked pasta, baked potatoes) held refrigerated for more than 24 hours were not properly date marked.
The growth of harmful organisms is slowed but not stopped by refrigeration. Over time, these organisms may increase their risk to public health. Datemarking these foods with an appropriate "use by" date shows when they are no longer safe to use. Refrigerated, ready-to-eat, TCS food held refrigerated for more than 24 hours shall be clearly marked at the time of preparation or the time the original container is opened to indicate the date or day to be consumed, sold, or discarded as required in this rule.
3717-1-03.5(E) / Consumer advisory.
Critical Repeat The consumer is not properly advised of the risk of consuming raw or undercooked animal foods because the consumer advisory statement at the bottom of the menu is not complete.
Except for whole-muscle intact beef steaks, if an animal food such as beef, eggs, fish, lamb, milk, pork, poultry, or shellfish is served or sold raw, undercooked, or without otherwise being processed to eliminate pathogens, either in ready-to-eat form, or as a raw ingredient in another ready-to-eat food, the license holder shall inform consumers of the significantly increased risk of consuming such food by way of a disclosure and reminder as described in this rule. Include a statement in the consumer advisory that informs consumers that the foods designated by an asterisk on the menu may be served raw or undercooked.
3717-1-04.1(C) / Food-contact surfaces - cleanability
Critical At the time of inspection, observed food contact surfaces of multiple food containers were cracked and/or chipped and therefore not easily cleanable.
Broken surfaces of food containers are not easily cleanable and may support the growth of harmful organisms which may be transferred to food. Multiuse food-contact surfaces shall be smooth and accessible for cleaning and inspection. PIC stated that new food containers were ordered. Discard all the cracked and/or chipped food containers.
3717-1-06.2(B) / Handwashing cleanser - availability.
Critical Observed no supply of hand cleaning liquid, powder or bar soap at the handwashing sink behind the bar.
Hands are a common vehicle for the transmission of pathogens to foods. The handwashing sink behind the bar shall be provided with a supply of hand cleaning liquid, powder, or bar soap so that employees may wash hands.

3717-1-04.4(A)(2) / Equipment components kept intact, tight, and adjusted
Repeat Observed the door seal for the downstairs reach in cooler was torn and hanging.
To ensure that the downstairs reach in cooler continues to function properly and can be maintained for easy cleaning, replace the damaged seal for the cooler door.

Observed that the seal for the ice cream freezer behind the bar was damaged.
Equipment components such as doors, seals, hinges, fasteners, and kick plates shall be kept intact, tight, and adjusted in accordance with manufacturer's specifications. To ensure that the ice cream freezer continues to function properly, replace the damaged seal. PIC stated that the door for the ice cream freezer would be replaced with a spare door from a freezer that will be taken out of service.
3717-1-04.5(A)(3) / Cleanliness of nonfood-contact surfaces of equipment.
Repeat Non-food contact surfaces of equipment are dirty. At the time of inspection, observed an accumulation of food residue, dust, and/or debris on multiple racks/shelves in the kitchen used for the storage of clean equipment, and on the door seals for multiple refrigeration units throughout the facility.
Surfaces that are dirty provide sources for contamination that may be transferred to food or clean equipment. Clean the dirty surfaces of this equipment and provide regular cleaning that maintains all equipment clean to sight and touch.
3717-1-06.2(E) / Handwashing signage.
Observed no handwashing sign posted at handwashing sink in the men's restroom.
A sign or poster that notifies food employees to wash their hands shall be provided at all handwashing sinks used by food employees.

Recommend posting conspicuous signage near the soda dispenser to ensure that all employees understand that the ice in the soda dispenser ice bin is used only for keeping condiments cold and as such can not be used in drinks.
