3717-1-05.1(E) / Backflow prevention device - design standard.
Critical At the time of inspection, observed improper backflow prevention device installed on a water supply system for F'Real milkshake machine.
A backflow or backsiphonage prevention device installed on a water supply system shall meet American Society of Sanitary Engineering (ASSE 1022) standards and as referenced in the Ohio plumbing code for installation, maintenance, inspection, and testing. Do not use this equipment until it is installed to code. PIC placed this equipment out of service at the time of inspection. Correct By: 17-Mar-2014
3717-1-05.3(C) / Backflow prevention.
Critical At the time of inspection, observed a direct connection between the sewage system and a drain originating from the F'real milkshake machines.
A direct connection may not exist between the sewage system and a drain originating from equipment in which food, portable equipment, or utensils are placed unless allowed by the Ohio building code. Do not use this equipment until it is installed to code. Eliminate this direct connection by maintaining at least 1" air gap for this drainage line. PIC took this equipment out of service at the time of inspection. Correct By: 17-Mar-2014

3717-1-06.4(B) / Cleaning - frequency and restrictions.
Observed a build-up of dirt and debris behind the F'Real milkshake machine at the time of inspection.
The physical facilities shall be cleaned as often as necessary to keep them clean, and cleaning shall be done during periods when the least amount of food is exposed.
