[?] A summary of the violations found during the inspection are listed below.

3717-1-06.1(C) / Floor and wall junctures - coved, and enclosed or sealed.
Observed the floor and wall junctures (corner by kitchen entrance, corner by handwashing sink, corner by food prep sink) were not properly coved or closed at the time of inspection.
To allow for easy cleaning, the floor and wall junctures shall be coved and closed to no larger than one thirty-second inch. Provide cove base for the missing areas.
3717-1-06.4(B) / Cleaning - frequency and restrictions.
Observed a build-up of dirt and debris on the floors in the dough walk-in cold holding unit (under shelving) and dough prep area at the time of inspection.
To prevent health hazards, the physical facilities shall be cleaned as often as necessary to keep them clean, and cleaning shall be done during periods when the least amount of food is exposed. The areas listed above shall be cleaned and maintained.

Surfaces such as cutting blocks and boards that are subject to scratching and scoring shall be resurfaced if they can no longer be effectively cleaned and sanitized, or discarded if they are not capable of being resurfaced.
Drains in receptacles and waste handling units for refuse, recyclables, or returnables shall have drain plugs in place.
A level 2 ODH certification in food safety training is required to be obtained for one operator of the facility by March 1, 2017.
