3717-1-02.4(C)(15) / PIC: duties - ensure employees are informed in a verifiable manner of their responsibility to report their health information
Critical Repeat Employees are not informed in a verifiable manner of their responsibility to report information about their health. Classroom employees that handle food shall be included in the FSO's sick policy.

3717-1-04.1(Y) / Temperature measuring devices.
Cold holding equipment thermometer was missing, located incorrectly, or not easily readable in the non-commercial refrigeration/freezer unit (staff fridge, temporarily being used until commercial fridge is capable of holding foods under 41 degrees F).

3717-1-04.8(E)(3) / Single-service articles and single-use articles - storage.
Single-service and single-use articles not protected from contamination. Observed paper towels removed from boxed and stored on open shelving in the dry storage room. Observed boxes of products stored on the floor in the dry storage room.

3717-1-04.1(KK) / Food equipment - certification and classification.
Equipment not approved by a recognized testing agency. Observed crockpot,griddle, refrigerator/freezer unit, refrigerator in the baby room, etc. that is not approved commercial equipment. PIC stated some items are not used. Items that are longer in use shall be removed from the facility

3717-1-04.5(D) / Nonfood-contact surfaces - cleaning frequency.
Non-food contact surface not cleaned at the required frequency. Observed the door gaskets on the warewashing machine and the air drying racks with an accumulation of food residue and debris.

Ensure dented cans are being rejected and if employees dent cans open the can and use the product.
Food employees (teachers) shall not wear jewelry on arms or hands during food preparation.
The violations documented shall be corrected and will be checked during next standard inspection.
