[?] A summary of the violations found during the inspection are listed below.

3717-1-02.4(C)(14) / PIC: duties - ensure employees are properly trained in food safety, including food allergy awareness.
Critical Person in charge did not ensure their employees are properly trained in food safety. The PIC was unable to demonstrate proper date marking procedures and the allergen information was not available for the deli items packaged with improper labels. Education was provided to the PIC to improve food safety procedures going forward.

3717-1-03.4(G) / Ready-to-eat, time/temperature controlled for safety food - date marking.
Critical Refrigerated, ready-to-eat, TCS foods not properly date marked in the deli cold holding unit (turkey, roast beef, and sub sandwiches). Education was provided to the PIC to improve date marking procedures.

3717-1-03.5(C)(2)(e) / Food labels - label information
Critical Missing and/or incomplete allergen declaration(s). Items packaged in the deli (deli salads, cheese, and sub sandwiches) do not list the allergens properly on the label. Education was provided to the PIC about proper label requirements.

3717-1-02.4(A)(2) / PIC - Level Two Certified Manager
Repeat Facility does not have an employee with level two certification in food protection.

3717-1-03.5(C)(2) / Food labels - packaged food
Repeat Label on a food packaged in-house did not contain all required information. Items packaged in the deli (deli salads, sub sandwiches, peanuts, cheese) do not contain all required information (sub-ingredients and address).

3717-1-04.1(H) / Nonfood-contact surfaces - cleanability
Nonfood-contact surfaces not easily cleanable. Shelves in the deli cooler and wooden shelves in the dry storage room are not easily cleanable.

3717-1-06.4(B) / Cleaning - frequency and restrictions.
Facility not maintained clean. There are multiple areas of the facility that are not clean (walk in cold holding unit, bathroom, dry storage room, storage cabinets, display cold holding unit).

3717-1-06.4(N) / Maintaining premises - unnecessary items and litter.
Unnecessary or nonfunctional items and /or litter on premises. There is litter behind the building. There is a large amount of old equipment on the premises (sandwich conveyer in the deli, cold holding unit in the deli, coolers in the dry storage room, and unnecessary items behind the building).

3717-1-06.4(A) / Repairing.
Repeat Physical facilities not maintained in good repair. There are multiple areas of the facility that are in disrepair (papertowel dispenser, damaged ceiling tiles in the retails space, and the floors, walls, ceilings, and floor junctures in and around the bathroom).
