[?] A summary of the violations found during the inspection are listed below.

3717-1-05.1(J) / Backflow prevention device - when required.
Critical Observed no approved backflow prevention device on the plumbing system for the rinse feature on the keg system at the time of inspection.
To prevent contamination, a plumbing system shall be installed to preclude backflow of a solid, liquid, or gas contaminant into the water supply system at each point of use at the FSO or RFE, including on a hose bibb if a hose is attached or on a hose bibb if a hose is not attached and backflow prevention is required by the Ohio building code, by: (1) Providing an air gap as specified under paragraph (D) of this rule; or (2) Installing an approved backflow prevention device as specified under paragraph (E) of this rule.
3717-1-05.3(C) / Sewage and other liquid waste: Backflow prevention.
Critical Observed a direct connection between the sewage system and a drain originating from equipment (rinse feature on top of keg system) in which food, portable equipment, or utensils are placed at the time of inspection.
To prevent health hazards, a direct connection may not exist between the sewage system and a drain originating from equipment in which food, portable equipment, or utensils are placed. Provide an air gap for the drainage line to meet the requirements of this rule.

3717-1-06.1(I) / Light bulbs - protective shielding.
Observed light bulbs that were not properly shielded or coated where required (above the bar area) at the time of inspection.
To prevent health hazards, light bulbs shall be shielded, coated, or otherwise shatter-resistant in areas where there is exposed food, clean equipment, utensils, or linens, or unwrapped single-service or single-use articles and does not apply in areas used only for storing food in unopened packages. Provide documentation light bulbs are shatter resistant or provide shielding for the light bulbs.
3717-1-06.2(E) / Handwashing signage.
Observed no handwashing sign posted at the handwashing sink used by employees in the bar area at the time of inspection.
To encourage frequent handwashing, a sign or poster that notifies food employees to wash their hands shall be provided at all handwashing sinks used by food employees. Provide a hand washing sign at the bar sink to meet the requirements of this rule.

Food must be supplied by an approved source. Recommend using licensed mobile food trucks.
If there is an event a cater can operate. If there is not an event the food needs to come from a state registered processor.
Recommend providing a paper towel dispenser for the hand sink at the bar area.
Utility service lines and pipes may not be unnecessarily exposed, may not be installed on the floor, and shall be installed so they do not obstruct or prevent cleaning of the floors, walls, or ceilings.
As discussed the employee health reporting policy shall be in written form and available for review.
A diarrheal and vomit clean-up procedure shall be posted as a written document and the proper disinfectant shall be available.
All violations documented will be checked for proper corrective action during the next standard inspection.
