3717-1-05.3(C) / Sewage and other liquid waste: Backflow prevention.
Critical Repeat Observed a direct connection between the sewage system and a drain originating from equipment in which food, portable equipment, or utensils are placed. Observed the facility was not provided with a designated, properly installed sink for preparing ready to eat foods at the time of inspection.
To prevent health hazards, a direct connection may not exist between the sewage system and a drain originating from equipment in which food, portable equipment, or utensils are placed unless allowed by the Ohio building code. Provide a food preparation sink that is properly installed with a in directly connected drainage to prevent contamination to foods. Until this sink is provided, all foods that need to be washed shall be washed at the Stanton Middle school kitchen.

3717-1-04.5(A)(3) / Cleanliness of nonfood-contact surfaces of equipment.
Corrected During Inspection Observed a spoiled milk odor and a milk spill inside the milk cold holding unit at the time of inspection.
To prevent health hazards, nonfood-contact surfaces of equipment shall be kept clean. The interior of the milk cooler shall be clean and maintained. A food employee cleaned the spill.
3717-1-06.1(M) / Outer openings - protected.
Repeat Observed an outer opening door without proper protection against the entry of insects and rodents. Observed back door to the kitchen was not self-closing at the time of inspection.
To prevent contamination, outer openings of an FSO shall be protected against the entry of insects and rodents as specified in this rule. Provide the back door to kitchen to be self-closing, self latching and tight-fitting.
3717-1-06.4(A) / Repairing.
Observed multiple stained ceiling tiles at the time of inspection.
To prevent health hazards, the physical facilities shall be maintained in good repair. Replace stained ceiling tiles with smooth and easily cleanable ceiling tiles.
