3717-1-05.1(E) / Backflow prevention device - design standard.
Critical Observed a hose connected to the water supply from the service sink with no backflow prevention device installed.
To prevent backsiphonage of water from the service sink to the water supply, an approved backflow device shall be installed. A backflow or backsiphonage prevention device installed on a water supply system shall meet American Society of Sanitary Engineering (ASSE) standards and as referenced in the Ohio plumbing code for installation, maintenance, inspection, and testing.
3717-1-05.1(L) / Handwashing sinks - location and placement.
Critical The Handwashing sink(s) are not located to allow convenient use by employees. There is no handwashing sink located behind the left side bar.
Proper handwashing is an important means of limiting illness. Having a conveniently located handwashing sink behind the left side bar encourages proper handwashing and makes handwashing more likely to occur. A handwashing sink shall be located to allow convenient use by employees in food preparation, food dispensing, and warewashing area, and in, or immediately adjacent to, toilet rooms.
3717-1-05.1(O)(1) / Using a handwashing sink - accessible at all times
Critical The handwashing sink was not easily accessible. A beverage cooler is obstructing the approach to the handwashing sink from the right side bar of the facility.
Because proper handwashing is an important means of limiting illness, nothing must obstruct the approach to a handwashing sink. A handwashing sink shall be maintained so that it is accessible at all times for employee use. Move the beverage cooler so that it does not obstruct the handwashing sink, or provide an additional fully provisioned handwashing sink behind the left side bar that is conveniently located for employee use.
3717-1-07(B) / Working containers - common name.
Critical Observed working containers of poisonous or toxic chemicals that were not properly labeled with the name of the material.
Working containers used for poisonous or toxic materials such as cleaners and sanitizers taken from bulk supplies shall be clearly and individually identified with the common name of the material.

3717-1-06.1(A) / Floors, walls, and ceilings.
Floors, walls, and/or ceilings were not smooth and easily cleanable. The interior surfaces of the walk in cooler are constructed of materials that are soft and/or porous (insulation/particle board).
Floors, floor coverings, walls, wall coverings, and ceilings shall be designed, constructed, and installed so they are smooth and easily cleanable. Improve the surfaces of the walk in cooler as required by this rule.
3717-1-06.4(A) / Repairing.
The physical facilities are not being maintained in good repair. At the time of inspection, observed drywall around the service sink was deteriorated and floor tiles behind the left side bar were damaged.
The physical facilities shall be maintained in good repair. Repair the walls around the service sink and floor tiles behind the bar so that these surfaces are maintained smooth, durable, and easily cleanable.

Air gaps have been provided for drain lines from ice bins behind right and left bars.
Discussed maintenance of self service popcorn station. An employee stationed near the popcorn during operating hours keeps this area clean. Discussed proper handwashing and warewashing procedures. Ensure that equipment is sanitized for 60 seconds at a concentration that is between 200 ppm and 400 ppm when a quaternary ammonia sanitizer is used.
Maintain wiping cloths submerged in buckets of sanitizer between uses.
