3717-1-05.3(C) / Sewage and other liquid waste: Backflow prevention.
Critical Corrected During Inspection Direct connection between the sewage system and a drain originating from equipment in which food, portable equipment, or utensils are placed. The ice machine drain did not have a proper air gap above the floor drain. The PIC was able to make a temporary repair to correct the violation at time of inspection.

3717-1-02.3(A) / Food contamination prevention - eating, drinking, or using tobacco.
Corrected During Inspection An employee drink was stored without a lid in the kitchen.

3717-1-06.4(B) / Cleaning - frequency and restrictions.
Floors are not clean in the kitchen in the hard to reach areas under equipment.
