Critical Observed there was no test kit available for measuring the concentration of the sanitizer at the time of inspection.
A test kit or other device that accurately measures the concentration in PPM (mg/L) of sanitizing solutions shall be provided.
3717-1-05.1(S)(1) / Plumbing system - maintained in good repair.
Critical Observed the cold water handle for the handwashing sink in the warewashing area was in poor repair at the time of inspection.
A plumbing system shall be repaired according to the Ohio building code. Repair the cold water handle to the handwashing sink so that it functions properly. Correct By: 08-Apr-2014
3717-1-05.3(C) / Backflow prevention.
Critical Observed a direct connection at the time of inspection between the sewage system and a drain originating from equipment (ice bin and ice machine) in which food, portable equipment, or utensils are placed.
A direct connection may not exist between the sewage system and a drain originating from equipment in which food, portable equipment, or utensils are placed unless allowed by the Ohio building code. To protect against contamination, eliminate this direct connection.
3717-1-07.1(J) / Restricted use pesticides - criteria.
Critical Observed an unapproved restricted use pesticide being used at the time of inspection.
Restricted use pesticides shall meet the requirements specified in 40 C.F.R. 152 Subpart I and be applied as specified under paragraph (C)(3) of this rule. Remove the rodent bait pellets from the premises and contact a licensed pest control agency for pest removal.
