3717-1-05.3(C) / Sewage and other liquid waste: Backflow prevention.
Critical Corrected During Inspection Direct connection between the sewage system and a drain originating from equipment in which food, portable equipment, or utensils are placed. The soda machine drain was not properly air gapped. The PIC was able to repair the drain once the violation was identified.

3717-1-04.4(A)(2) / Equipment components kept intact, tight, and adjusted
Equipment components are not intact, tight or properly adjusted. The reach in cold holding unit along the main preparation line is not operational. The unit is not in use and the PIC states that a maintenance repair order has been requested.

3717-1-04.5(A)(3) / Cleanliness of nonfood-contact surfaces of equipment.
Non-food contact surfaces of equipment are unclean. The hot holding cabinets were not clean.
