3717-1-02.4(A)(2) / PIC - Manager Certification
Repeat Facility does not have an employee with manager certification in food protection.

3717-1-04.4(A)(2) / Equipment components kept intact, tight, and adjusted
Equipment components are not intact, tight or properly adjusted. Observed three-door freezer at the far end of the store in disrepair.

3717-1-04.5(D) / Nonfood-contact surfaces - cleaning frequency.
Non-food contact surface not cleaned at the required frequency. Observed front counter area specifically near the hand sink not maintained clean.

3717-1-06.4(A) / Repairing.
Repeat Physical facilities not maintained in good repair. Observed multiple damaged and stained ceiling tiles throughout the facility.

3717-1-06.4(N) / Maintaining premises - unnecessary items and litter.
Repeat Unnecessary or nonfunctional items and /or litter on premises. Observed paint supplies and equipment and other items that are not used for the operation of the facility. Observed the back room full of old equipment and no longer in use.

3717-1-06.2(I)(1) / Lighting - intensity (10 FC)
Repeat Light intensity less than ten foot candles in required areas. Obsered measured light intensity in the back of the store at 3-4 foot candles. Observed multiple light bulbs that need changed out and are no longer working.

Complaint on 09-Sep-2020 :
A standard inspection was conducted and also inspection conducted in regards to a complaint.

Per complainant: Employees are not wearing mask and not requiring customers to wear masks. No protective barriers and social distancing markers/guidance.

Complaint findings: Observed no masks being worn by both employees at the beginning of inspection. Employees put masks on and stated they wear them throughout the day. Observed no signage regarding occupany in the store, no distancing markers at the front counter, or signage regarding face coverings. Example posters will be attached with this report that can be posted. Observed customers without masks being asked to put masks on at the time of inspection.

Please continue to let customers know that it is required to wear face coverings while in the store.

Per discussion with PIC, they have not sold pizza and are looking to go a different route. Contact the Health department for an inspection prior to selling any hot foods.
The items listed below and on this report shall be completed prior to selling hot foods:
1. Approval from fire department.
2. Back room needs to be cleared out to have access to 3-compartment sink and mop sink.
3. Install hand sink in the back warewashing room.
4. Unnecessary items including broken equipment shall be removed or repaired (slicer, ice machine, etc.).
5. Chemicals stored in the open glass container (tobacco products, pipes, etc.) in the pizza prep area shall be relocated away from operation.
6. Storage for clean and dirty utensils and equipment shall be provided at the three-compartment sink.

The violations documented shall be corrected and will be checked during next standard inspection.
