Comments
This inspection was completed per the request of the new owner regarding the Facility Review to change the licensee and issue a new Risk Level IV FSO license. The follow shall be completed for approval to be issued a license:
1) Replace the damage ceiling tiles in the hallway to the rest-rooms.
2) Provide a properly located temperature measuring device with each cold holding or hot holding unit.
3) Clean hard to reach floors through out the bar area. Pull out beer coolers and clean.
4) Clean the outdoor refuse area.
5) Provide a better means to store employees clothing and belongs. Nothing shall be stored around the electrical panels in the hot heater room.
6) Clean under ice bin and around the service bar.
7) Provide a proper air gap on the drainage from the ice bin and overflow drain from the soda dispensing equipment at the beverage line. Drains current are directing flow onto the floor.
8) Move foods and single service articles from under the hand washing on the line.
9) Provide proper paper towels, soap, and hand washing signage at each designated hand washing sink.
10) Provide a proper Time Instead of Temperature procedure or "TILT" for the use of ice as a external cold holding of foods. If ice is a normal means to cold hold foods during operational hours then additional cold holding will be required.
11) Caulk and reseal all wall mounted sinks and equipment to the wall. (3 compartment, hand washing sinks, dirty and clean tables of dish, etc.)

Once these items have been corrected, contact the KCHD for a final pre-licensing inspection. Application for a FSO license will be provided at the next inspection.

Lighting throughout the FSO is in need of improvement as follows:
1) Increase lighting to 20 foot candles of light at the hand washing sinks in both the men's and women's rest-room. Both were observed to be below 12 foot candles.
2) Additional light is require on the food preparation line. Pizza and salad line was observed to be below 15 and in some locations 10 foot candles. This shall be increased to 50 foot candles. Hot foods section and the light over the cooking equipment under hood shall be increase to at least 50 foot candles.

The lighting can be corrected at later date. Please provide written documentation of the owners Corrective Action Plan on the light issue. Please provided a deadline to when the owner plans to have this corrected. If any items 1-12 need additional time, they may be added to the the Corrective action plan letter.
