[?] A summary of the violations found during the inspection are listed below.

3717-1-03.4(G) / Ready-to-eat, time/temperature controlled for safety food - date marking.
Critical Corrected During Inspection Refrigerated, ready-to-eat, TCS foods not properly date marked. Observed large container of hard boiled eggs without a proper date mark for 7 day discard. PIC was using the manufacturer date of October 1st as the date to use by. Once open, the product shall be dated for a 7 day discard and properly discarded. Food was discard at the time of inspection.

Critical Direct connection between the sewage system and a drain originating from equipment in which food, portable equipment, or utensils are placed. Observed the drain pipe from the ice machine to not be provided with a proper air gap. Repair the pipe to be properly air gapped.

3717-1-03.2(Q) / Food storage - preventing contamination from the premises.
Improper storage of food items. Observed box of frozen foods on the floor of the walk-in freezer under a leaking condensation drainage line. Foods shall not be stored on the floorer under leaking condensation drainage pipes.

Equipment components are not intact, tight or properly adjusted. Observed the Velvet ice cream display freezer was damaged. Repair or replace equipment as needed.

Floor and wall junctures not properly coved. Observed floor and wall junctures throughout the FSO were not properly with a proper coved finish. Wall corner next to the customer self service beverage dispenser was damage with missing base board.

Physical facilities not maintained in good repair. Observed back hand sink by 3 compartment sink needs recaulked/sealed to wall.

Comments
1) Adjust prep coolers to be able to hold all TCS foods at 41 degrees F or below through busy serving times.
2) Each shift is required to have at least one person with PIC certifications. At least one supervisor with ODH managers certification in Food Protection shall be assigned to this location (not required to be present at inspection).
3) If food handlers are delivering the foods to the hands of customers, gloves shall be considered contaminated. Gloves shall be change and hands are to washed before new gloves are donned. One person shall be handling the delivery of the foods to waiting customers.
4) Ensure staff is properly washing, rinsing, and ware washing deli slicer at least every 4 hours, before and after each use.
5) Proper hair restraint does require pony tails and other loose hair to be contained with a covering.
6) Ensure the lettuce and cut leafy greens can be verified washed and ready to eat before assuming. If not able to be verified, the cut leafy greens shall be washed before served.

Overall, the facility was clean and organized. Most of the TCS was proper date marked. No food temperature issues observed. PIC was certified and knowledgeable.
