3717-1-02.4(B)(2)(h) / PIC: Demonstration of Knowledge - Describing relationship between prevention of foodborne illness and management of contamination, hand contact with food, hand washing, and clean facility
Critical PIC unable to demonstrate knowledge of preventing foodborne illness by controlling contamination, personal hygiene, and facility maintenance. Cross contamination from raw meat was observed with contaminated glove usage and by not separating raw utensils. Education was provided to the PIC to improve proper food safety procedures.

3717-1-02.2 (C ) / Hands and arms: when to wash
Critical Food employee(s) did not wash hands when required. The PIC contaminated ready to eat food and equipment with gloves used to handle raw meat. Education was provided to the PIC to improve proper food safety procedures (hand washing, glove usage, cross contamination, etc.).

3717-1-04.5(B)(1) / Equipment food-contact surfaces and utensils - cleaning frequency.
Critical Food equipment surfaces not cleaned at required frequency. The PIC used the same cooking utensils to handle raw meat as cooked meat without washing or switching utensils. Education was provided to the PIC to eliminate cross contamination practices.

3717-1-03.4(G) / Ready-to-eat, time/temperature controlled for safety food - date marking.
Critical Refrigerated, ready-to-eat, TCS foods not properly date marked. Tomatoes and lettuce were not date marked. Education was provided to the PIC to improve food safety knowledge going forward.

3717-1-07(B) / Poisonous or toxic materials: Working containers - common name.
Critical Working containers of poisonous or toxic materials not properly labeled. Multiple chemical spray bottles were not labeled. The PIC was instructed to label all bottles.

3717-1-03.2(D) / Food storage containers - identified with common name of food.
Repeat Working food containers not properly labeled. Bottles of sauce and oil were not labeled in the preparation area.

3717-1-03.2(K) / In-use utensils - between-use storage.
In-use utensils improperly stored. Chicken tongs were stored improperly in a sanitizer bucket in between uses near the freezer.

3717-1-04.4(A)(2) / Equipment components kept intact, tight, and adjusted
Equipment components are not intact, tight or properly adjusted. A metal utensil used to cook food on the grill is severely chipped.

3717-1-04.2(I) / Sanitizing solutions - testing devices.
No sanitizer test kit available.

3717-1-04.5(A)(3) / Cleanliness of nonfood-contact surfaces of equipment.
Repeat Non-food contact surfaces of equipment are unclean (cold holding equipment, sinks, freezer, cooking equipment, exhaust hoods, storage racks and shelving).

3717-1-06.4(N) / Maintaining premises - unnecessary items and litter.
Repeat Unnecessary or nonfunctional items stored in the basement (old equipment).

3717-1-06.4(B) / Cleaning - frequency and restrictions.
Facility not maintained clean. The hard to reach areas of floor are not clean under equipment in the kitchen.
