3717-1-03.4(G) / Ready-to-eat, time/temperature controlled for safety food - date marking.
Critical Observed refrigerated, ready-to-eat, TCS foods (bags of salad greens and spinach) at the time of inspection that were held refrigerated for more than 24 hours were not properly date marked.
To prevent foodborne illness, refrigerated, ready-to-eat, TCS food held at a temperature of 41°F or less for more than 24 hours shall be clearly marked at the time of preparation or the time the original container is opened to indicate the date or day, that is a maximum of seven days, by which the food shall be consumed, sold, or discarded. 3717-1-04.2(B)(1) / Manual warewashing - three compartment sink required.
Critical Repeat Observed the facility was not equipped with a three-compartment sink for the manual washing, rinsing, and sanitizing of utensils at the time of inspection.
To prevent contamination, a sink with at least three compartments shall be provided for manually washing, rinsing and sanitizing equipment and utensils.
3717-1-06.4(K) / Controlling pests.
Critical Observed mouse feces in the back corner of the dry storage area that indicated the presence of live rodents in the facility at the time of inspection.
To prevent illness, the presence of insects, rodents, and other pests shall be controlled by: Routinely inspecting incoming shipments of food and supplies; routinely inspecting the premises for evidence of pests; Using methods, if pests are found, such as trapping devices or pesticides used according section (C), (K) and (L) of rule 3717-1-07.1 of this code; and eliminating harborage conditions. A professional pest control agency with licensed personnel shall be hired to eliminate pests in this facility. Correct By: 03-Feb-2015

3717-1-03.2(Q) / Food storage - preventing contamination from the premises.
Observed improper storage of food items (bags of bulk grains, carrots, and other foods in storage or on display) on or within 2 inches of the floor at the time of inspection.
Food shall be protected from contamination by storing the food in a clean, dry location, where it is not exposed to splash, dust, or other contamination, and at least six inches above the floor. Correct By: 03-Feb-2015
3717-1-03.5(C)(2) / Food labels - packaged food
Observed labels on a some packaged foods in the facility did not contain all required information at the time of inspection.
To adequately inform a consumer of the contents of packaged food, a food label information shall include: (a) The common name of the food, or an adequately descriptive identity statement; (b) If made from two or more ingredients, a list of ingredients in descending order of predominance by weight, including a declaration of artificial color or flavor and chemical preservatives, if contained in the food; (c) An accurate declaration of the quantity of contents; (d) The name and place of business of the manufacturer, packer, or distributor; (e) The name of the food source for each major food allergen contained in the food, unless listed in the ingredients; (f) Nutritional information when required by law; and (g) Proper Labeling on container of salmonid fish containing canthaxanthin.
3717-1-04.1(KK) / Food equipment - certification and classification.
Observed equipment (household freezers) in the facility at the time of inspection that was not approved by a recognized testing agency.
To prevent health hazards, food equipment that is acceptable for use in a RFE shall be approved by a recognized food equipment testing agency. Remove this unapproved equipment. Please contact the City of Kent Health Department before adding any equipment to this facility.
3717-1-05.1(I) / Service sink - number.
Repeat Observed no service sink or curbed cleaning facility at the time of inspection as required.
At least one service sink or one curbed cleaning facility equipped with a floor drain shall be provided and conveniently located for the cleaning of mops or similar wet floor cleaning tools and for the disposal of mop water and similar liquid waste.
3717-1-05.4(B) / Outdoor storage surface.
Repeat Observed the outdoor refuse storage surface for the refuse container was not properly constructed, and sloped to drain at the time of inspection.
A properly constructed storage surface that can be thoroughly cleaned is less likely to attract pests. Provide an outdoor storage surface for refuse, recyclables, or returnables that is constructed of nonabsorbent material and is smooth, durable, and sloped to drain.
3717-1-06.1(C) / Floor and wall junctures - coved, and enclosed or sealed.
Repeat Observed the floor and wall junctures are not properly coved or closed in the back storage area at the time of inspection.
Dirt, food, and other debris may accumulate in floor and wall junctures that are not properly coved or closed. In RFE's in which cleaning methods other than water flushing are used for cleaning floors, the floor and wall junctures shall be coved and closed to no larger than one thirty-second inch; where water flush cleaning methods are used, shall be provided with drains and be graded to drain, and the floor and wall junctures shall be coved and sealed.
3717-1-06.1(I) / Light bulbs - protective shielding.
Observed light bulbs in the walk in cooler that were not properly shielded or coated at the time of inspection.
To protect against contamination in the event a light bulb may break, light bulbs shall be shielded, coated, or otherwise shatter-resistant in areas where there is exposed food, clean equipment, utensils, or linens, or unwrapped single-service or single-use articles.
3717-1-06.1(M) / Outer openings - protected.
Repeat Observed the back door of the facility was not equipped with a self-closing mechanism at the time of inspection for proper protection against the entry of insects and rodents.
To prevent contamination, outer openings of a RFE shall be protected against the entry of insects and rodents by solid, self-closing, tight-fitting doors. Provide the back door with a properly functioning self-closing mechanism.
