[?] A summary of the violations found during the inspection are listed below.

3717-1-02.4(C)(14) / PIC: duties - ensure employees are properly trained in food safety, including food allergy awareness.
Critical Person in charge did not ensure their employees are properly trained in food safety. Observed italian wedding soup in the hot holding equipment and employees are unsure what is in the product. Provide ingredient list with soups that are made from another location on campus.

3717-1-02.4(C)(11) / PIC: duties - ensure employees are proplery sanitizing cleaned equipment and utensils
Critical Corrected During Inspection PIC not ensuring employees are properly sanitizing equipment and utensils and monitoring of sanitizer parameters. Observed high-temp dish machine being used and the screen displaying: add detergent and add rinse aid. The three-compartment sink shall be used until equipment can be repaired to work properly.

3717-1-02.4(C)(15) / PIC: duties - ensure employees are informed in a verifiable manner of their responsibility to report their health information
Critical Employees are not informed in a verifiable manner of their responsibility to report information about their health. An example employee health reporting agreement will be attached to this report that can be used.

3717-1-03.4(F)(1)(b) / Time/temperature controlled for safety food - cold holding.
Critical TCS foods not being cold held at the proper temperature. Observed milk and cream measured above 41 degrees F (55 degrees F) at the customer self-service area.

3717-1-03.4(I)(1) / Time as a public health control - written procedures
No written procedures for time as a public health control. The current license does not allow for a 4 hour time instead of temperature procedure. If the FSO would like to implement an approved tilt procedure, an increase in risk category (3 to a 4) will need to occur for next licensing year.

3717-1-04.2(C) / Drainboards.
Repeat Insufficient storage space provided for soiled and clean items.

3717-1-04.5(A)(3) / Cleanliness of nonfood-contact surfaces of equipment.
Observed an accumulation of debris on the gaskets of the prep refrigeration unit. Observed the lid and exterior of the creamer/milk containers above the warewashing sink unclean. Observed the interior door gasket of the warewashing machine not maintained clean.

3717-1-06.4(N) / Maintaining premises - unnecessary items and litter.
Repeat Unnecessary or nonfunctional items on premises. Observed equipment and utensils that are no longer being used stored above the three-compartment sink and next to the hand sink.

Comments
A food defense plan is required to ensure the FSO is protected. Plan shall include details on how the staff will ensure the protection of the FSO when not in operation and the Architectural building is open to the public. Plans for set-up and closure that cover the steps the staff with take to ensure the safety is needed.

Recommend providing shelf stable creamers or storing the milk/creamers in cold holding.
A 4 hour time procedure may be implemented but shall be approved and the food license would need to be increased to a risk level 4.

Flooring throughout shall be maintained clean. Ensure the FSO has access to the building's service sink.
If water filter are used for water supply lines, they shall be maintained and serviced as stated by the manufacturer, documentation shall be readily available.
Post a ODH choking poster at the location.
Post original license at the location.

The violations documented shall be corrected and will be checked during the scheduled follow-up inspection.
