[?] A summary of the violations found during the inspection are listed below.

3717-1-05.3(C) / Sewage and other liquid waste: Backflow prevention.
Critical Repeat Observed a direct connection between the sewage system and a drain originating from equipment in which food, portable equipment, or utensils are placed. Observed a directly connected drain on the 3-compartment sink (used for washing fruits and vegetables) at time of inspection.
To prevent health hazards, a direct connection may not exist between the sewage system and a drain originating from equipment in which food, portable equipment, or utensils are placed unless allowed by the Ohio building code for: a warewashing machine between its waste outlet and a floor drain when the machine is located within five feet (1.5 meters) of a trapped floor drain and the machine outlet is connected to the inlet side of a properly vented floor drain trap; or a warewashing and culinary sink. Provide a properly designed food prep sink.

3717-1-03.2(Q) / Food storage - preventing contamination from the premises.
Observed improper storage of food items in the walk-in freezer at the time of inspection.
To prevent contamination, food shall be protected from contamination by storing the food in a clean, dry location, where it is not exposed to splash, dust, or other contamination, and at least six inches above the floor. Discontinue storing boxes directly on the floor.
3717-1-04.5(A)(3) / Cleanliness of nonfood-contact surfaces of equipment.
Observed non-food contact surfaces of equipment with an accumulation of food debris and dirt at the following locations at the time of inspection: 1) floors and walls of walk-in cold holding unit. 2) floors in the walk-in freezer. 3) Interior of drawers under the topping preparation area.
To prevent health hazards, nonfood-contact surfaces of equipment shall be kept clean. Clean and maintain the equipment surfaces listed above. Bottom shelves in walk-ins shall be maintained up off the floor (atleast 6 inches).
3717-1-04.8(E) / Equipment, utensils, linens, single-service articles, and single-use articles - storage.
Corrected During Inspection Observed single-service articles (plastic spoons) were not properly stored in the dry storage area (uncovered, exposed) at the time of inspection.
To prevent contamination, cleaned equipment and utensils, laundered linens, single-service articles, and single-use articles shall be properly stored as described in this rule. (3) Single-service and single-use articles shall be stored as specified under paragraph (E)(1) of this rule and shall be kept in the original protective package or stored by using other means that afford protection from contamination until used.
3717-1-05.1(S)(2) / Plumbing system - maintained in good repair.
Repeat Observed the plumbing system on the water filters was not properly maintained near the back warewashing room at the time of inspection. A fitting for the plumbing system was leaking water onto the floor.
To prevent health hazards, a plumbing system shall be properly maintained. Repair the plumbing system listed above.
3717-1-05.3(I) / Other liquid wastes and rainwater.
Repeat Observed improper draining of condensation and other nonsewage liquids for the walk-in cold holding unit and for the ice bin next to the salad prep cold holding unit at the time of inspection. Condensation lines were missing and were draining into buckets.
To prevent health hazards, condensate drainage and other nonsewage liquids and rainwater shall be drained from point of discharge to disposal according to law. Provide proper drainage for the walk-in unit and ice bin (air gaps and directed to floor drains).
3717-1-06.1(A) / Floors, walls, and ceilings.
Observed floors behind the ice machine (location of old freezers) were not smooth and easily cleanable at the time of inspection.
To allow for easy cleaning, Floors, floor coverings, walls, wall coverings, and ceilings shall be designed, constructed, and installed so they are smooth and easily cleanable. Repair or replace flooring behind the ice machine.
3717-1-06.1(C) / Floor and wall junctures - coved, and enclosed or sealed.
Repeat Observed multiple floor and wall junctures throughout the facility that were not properly coved or closed at the time of inspection.
To prevent health hazards, the floor and wall junctures shall be coved and closed to no larger than one thirty-second inch; where water flush cleaning methods are used, shall be provided with drains and be graded to drain, and the floor and wall junctures shall be coved and sealed. Floor and wall junctures shall be properly coved.
3717-1-06.4(B) / Cleaning - frequency and restrictions.
Observed a build-up of dirt and food debris on the floors and wall behind the ice machine and the floors and wall around the washing machine at the time of inspection.
To prevent health hazards, the physical facilities shall be cleaned as often as necessary to keep them clean, and cleaning shall be done during periods when the least amount of food is exposed. Clean and maintain the floors listed above.

Ensure handwashing occurs when arriving to work and before starting a task.
To prevent contamination, unless wearing intact gloves in good repair, a food employee may not wear fingernail polish or artificial nails when working with exposed food.
A plumbing system shall be installed to preclude backflow of a solid, liquid, or gas contaminant into the water supply system at each point of use at the FSO (ice machine, coffee machine, slushy machine, etc.).
Milk is considered a TCS food and once opened, it shall be dated to ensure it is used or discarded within 7 days.
Ensure the catch trays for the new cooking equipment (conveyor broiler) are warewashed every 4 hours.
Drainage lines off ice machines (bin and condenser) shall be separate and contain air gaps.

The violations documented on this report shall be corrected as soon as possible and will be checked during next standard inspection.
