3701-21-03 OAC / Facility layout and equipment specifications for FSO
Observed the FSO constructed or altered the facility without prior licensor approval at the time of inspection.
To prevent foodborne illness, no person, firm, association, organization, corporation, or government operation shall construct, install, provide, equip, or extensively alter a food service operation until the facility layout and equipment specifications have been submitted to and approved in writing by the licensor. Please submit plans identifying the alterations to the facility. Permits may need to be obtained for the installations that occurred, contact building department as soon as possible.
3717-1-04.8(E) / Equipment, utensils, linens, single-service articles, and single-use articles - storage.
Observed cleaned utensils not properly stored on the dining room tables at the time of inspection.
To prevent contamination, cleaned utensils shall be properly stored: (a) In a clean, dry location; (b) Where they are not exposed to splash, dust, or other contamination; and (c) At least six inches (fifteen centimeters) above the floor. Discontinue storing tablware unprotected on the dining room tables.
