[?] A summary of the violations found during the inspection are listed below.

3717-1-03. / Food - safe, unadulterated, and honestly presented
Observed food (hot pockets, frozen pizzas, Big Az sandwiches, etc.) that was unsafe or adulterated in the Hussman Freezer at the time of inspection.
To prevent foodborne illness, food shall be safe, unadulterated, and honestly presented. The food items in the Hussman Freezer shall be removed from the facility.
3717-1-04.4(A)(1) / Equipment - good repair and proper adjustment.
Observed the Hussman freezer that was not maintained in good working order (broken) at the time of inspection.
To prevent health hazards, equipment shall be maintained in a state of repair and condition that meets the requirements specified under rules 3717-1-04 and 3717-1-04.1 of this code. Repair/replace the Hussman Freezer or remove from RFE. The PIC stated the freezer has a work order to repair the damaged unit.
3717-1-06.4(A) / Repairing.
Observed the physical facilities are not being maintained in good repair. The ceiling in the walk-in was leaking water at the time of inspection.
To prevent health hazards, the physical facilities shall be maintained in good repair. Food items shall not be stored under or near this area. Repair the ceiling to meet the requirements of this rule.
3717-1-06.4(B) / Cleaning - frequency and restrictions.
Observed a build-up of dirt and debris in the following locations: (1) The floors in the walk-in cold holding unit (under shelves). (2) The floors under and around pizza preparation cold holding unit/oven. (3) The condenser/blower in the walk-in cold holding unit.
To prevent health hazards, the physical facilities shall be cleaned as often as necessary to keep them clean, and cleaning shall be done during periods when the least amount of food is exposed. Clean and maintain the areas listed above.
3717-1-06.4(N) / Maintaining premises - unnecessary items and litter.
Repeat Observed unnecessary or nonfunctional items in the back storage room at time of inspection.
To allow for proper cleaning, remove the nonfunctional items from the storage room. The premises shall be free of Items that are unnecessary to the operation or maintenance of the RFE such as equipment that is nonfunctional or no longer used and litter.

Observed no risk level 3 activities at the time of inspection. There was no food preparation and only pre-packaged foods were being sold.
Per discussion with PIC, the facility may drop risk level for next licensing year. Please provide a letter stating this and also state there will be no risk level 3 activities at the RFE.
The facility shall remove any equipment or items not used in the operation.
All risk level 3 and risk level 4 operations are required to obtain a level 2 ODH certification in food safety training for one operator of the facility by March 1, 2017.
