Repeat Non-food contact surfaces of equipment are unclean (cold holding equipment, hot holding equipment, storage racks and shelving).

Repeat Toilet room did not have tight-fitting, self-closing door.

Repeat Facility not maintained clean. The floors throughout the kitchen are not clean in the hard to reach areas under equipment.
