Critical Upon review of local health department records, it was observed that this FSO was altered and equipment was added without prior licensor approval.
To prevent foodborne illness, no person, firm, association, organization, corporation, or government operation shall construct, install, provide, equip, or extensively alter a food service operation until the facility layout and equipment specifications have been submitted to and approved in writing by the licensor. Submit the required facility review plans to the Kent City Health Department. Correct By: 15-May-2015
