3717-1-06.1(M) / Outer openings - protected.
Repeat Outer back door without proper protection against the entry of insects and rodents (not self-closing).

3717-1-03.2(Y) / Miscellaneous sources of contamination.
Employee beverages were not being stored in designated locations away from food preparation. Ensure employee belongings are stored away from operation's products.

3717-1-04.8(G)(1) / Kitchenware and tableware - handling of single-service and single use articles.
Single-service or tableware (plastic spoons) not handled, displayed, or dispensed properly. Observed lip contact surfaces facing upward in container at the drive-thru window.

3717-1-04.5(A)(3) / Cleanliness of nonfood-contact surfaces of equipment.
Observation: Observed an accumulation of food residue and other debris on the following nonfood-contact surfaces of equipment: 1) Shelving in walk-in refrigeration. 2) Walls under the condensor and behind the shelving unit in the walk-in refigeration unit (cardboard shall not be used as a shelf liner). 3) Gaskets and handles for multiple freezer and refrigeration units (damaged gaskets shall be repaired).

Repeat Walls and ceilings not smooth and easily cleanable throughout the facility. Observed unfinished surfaces and missing ceiling tiles within the facility. Observed damaged paint and surfaces on the exterior of the walk-in refrigeration unit and the door for the walk-in freezer.
