3717-1-02.4(A)(2) / PIC - Manager Certification
Repeat Facility does not have an employee with manager certification in food protection.

3717-1-04.4(A)(2) / Equipment components kept intact, tight, and adjusted
Equipment components are not intact, tight or properly adjusted. Observed walk-in freezer in disrepair and empty. PIC stated the large amount of food in the unit was discarded. PIC is looking to get a small NSF freezer or a chest freezer which would only be temporarily until the walk-in can be repaired.

Discussed COVID-19 protocols (face coverings, social distancing, cleaning and sanitizing, sick policy, etc.) during the inspection with the person-in-charge.
Provided handouts for and to help with the mandatory operations and best practices outlined in the dine safe Ohio order.
Per the governor's orders, all employees should wear face coverings. A justification letter was provided during the inspection.

The violations documented shall be corrected and will be checked during next standard inspection.
