[?] A summary of the violations found during the inspection are listed below.

3717-1-02.2(C) / Hands and arms: when to wash
Critical Observed employees that did not wash hands in situations that specifically require them to do so (after touching bare human body parts, before starting work) at the time of inspection.
To prevent contamination, employees shall clean their hands and exposed portions of their arms after switching between working with raw food and with ready to eat food; before donning gloves to initiate a task that involves working with food; and after other activities that contaminate the hands.
3717-1-05.3(C) / Sewage and other liquid waste: Backflow prevention.
Critical Repeat Observed a direct connection between the sewage system and a drain originating from equipment in which food, portable equipment, or utensils are placed. Observed the facility was not provided with a designated a properly installed sink at the time of inspection for preparing ready to eat foods.
To prevent health hazards, a direct connection may not exist between the sewage system and a drain originating from equipment in which food, portable equipment, or utensils are placed unless allowed by the Ohio building code. Provide a food preparation sink that is properly installed with a in directly connected drainage to prevent contamination to foods. Until this sink is provided, all foods that need to be washed shall be washed at the Stanton Middle school kitchen.

3717-1-04.4(S)(1) / Single-service articles and single-use articles - use limitation.
Corrected During Inspection Observed re-use of single-use container (bread bowl) at the time of inspection.
To prevent health hazards, single-service and single-use articles may not be reused. Discontinue re-using containers that are designed to be re-used. The PIC removed the single-use container with a food grade container.
3717-1-06.1(M) / Outer openings - protected.
Repeat Observed an outer opening door without proper protection against the entry of insects and rodents. Observed back door to the kitchen was not self-closing at the time of inspection.
To prevent contamination, outer openings of an FSO shall be protected against the entry of insects and rodents as specified in this rule. Provide the back door to kitchen to be self-closing, self latching and tight-fitting.

Food preparation shall occur immediately after food items are taken out of cold holding.
Recommend that the lunch tables are sanitized before the first lunch period and between each lunch service.

The facility's current employee health policy has 5 of the 13 illnesses that the Ohio Food Code requires to be reported. Refer to the rule 3717-1-02 (A)(3) for the complete list of illnesses that shall be included.
