[?] A summary of the violations found during the inspection are listed below.

3717-1-05.3(C) / Backflow prevention.
Critical Observed that direct connections observed during the standard inspection at the facility on January 29, 2013 still exist.
A direct connection may not exist between the sewage system and a drain originating from equipment in which food, portable equipment, or utensils are placed unless allowed by the Ohio building code. To protect ice, ice bins, and ice machine from contamination, eliminate these direct connections.

3717-1-04.2(I) / Sanitizing solutions - testing devices.
At the time of inspection, PIC was unable to locate test kit for measuring the concentration of the sanitizer.
Provide a test kit or other device that accurately measures the concentration in PPM (mg/L) of sanitizing solutions and use the test kit to ensure sanitizing solutions are at the appropriate concentration.
3717-1-05.1(O)(2) / Using a handwashing sink - other uses prohibited.
Corrected During Inspection Ice cubes were observed in handwashing sink behind the first floor main bar at the time of inspection. PIC stated that this handwashing sink had been used as a dump sink.
A handwashing sink may not be used for purposes other than handwashing. PIC supplied a bucket that will be used for dump purposes. Empty this bucket regularly and maintain handwashing sink so that it is not used for any other purpose.
3717-1-06.1(I) / Light bulbs - protective shielding.
Observed lighting over ice machine was not properly shielded or coated as required.
To protect ice from potential contamination, lighting over ice machine shall be shielded, coated, or otherwise shatter-resistant.
3717-1-06.2(C) / Handwashing sinks - hand drying provision.
Observed no towels or hand drying device at the handwashing sinks behind each bar in the facility.
At the time of inspection, PIC provided paper towels for use at the handwashing sink behind the first floor main bar. Provide paper towels at each handwashing sink for employees to dry hands after washing their hands.
3717-1-06.4(A) / Repairing.
Observed damaged floor tiles with areas of exposed wood at first floor main bar and by door to ice machine room. The physical facilities are not being maintained in good repair.
The physical facilities shall be maintained in good repair such that theses surfaces are smooth, durable, and easily cleanable.
